When should I notify Activa after an accident or event occur?

Immediately notify Activa within 24hours by:
  • Filing a claim form online and submit or download it here(Claim form)
  • Contacting your Relationship Officer/ Agent.

What information do I need?

  • Your name, location address and phone contact
  • Names, addresses, location and phone contact of witnesses and or injured persons
  • General loss information (Date, Time, Place, Situation and Damages and/or Injuries)
  • Your policy number (if available)
  • Police Report (For Motor Accident and Theft/Burglary of property)
  • Fire Service Report (Fire damages)
  • Driver’s license copy(For Motor accident)
  • Pictures of accident (damages & or injuries)

What happens after I report a claim?

What should I do after suffering property damage?

If your property has been damaged:
  • Notify us immediately
  • If theft or vandalism caused the loss, notify the police.
  • If the loss is related to a bank electronic fund transfer or credit card, notify the financial institution.
  • If the loss is fire related notify the National Fire Service
  • If the loss id work related injuries notify the Labor Office

What should I do after an auto accident?

If you’ve been involved in an accident:
  • Notify Activa immediately
  • Notify the police.
  • Valid Drivers’ license
  • Present a copy of Valid Drivers’ license
  • Collect the descriptions and specific damages to all vehicles, as well as the names and addresses of all individuals and witnesses involved.
  • Promptly send Activa copies of any letters, notices or legal papers you receive about the accident. You may be asked to submit proof of loss documentation. If you received treatment for injuries, you may be asked to submit that information as well.
